Male Sexual Dysfunction — ED, Low Testosterone & Peyronie's Disease Treatment in Newmarket
Dr. Jerome Green MD, FRCSC — Urologic Surgeon
Erectile Dysfunction
Erectile dysfunction (ED) — the persistent inability to achieve or maintain an erection sufficient for satisfactory sexual activity — affects an estimated one in five Canadian men over the age of 40. While ED can be distressing, it is also highly treatable.
Dr. Green conducts a thorough evaluation including hormonal bloodwork and vascular assessment. Dr. Green will identify the underlying cause, which may include cardiovascular disease, diabetes, neurological disease, hormonal imbalance, psychological factors, or medication side effects. First-line treatment typically involves PDE5 inhibitors (sildenafil, tadalafil, vardenafil), which are effective for many patients. When oral medications are ineffective or associated with unwanted side effects, Dr. Green offers low intensity shock wave therapy, intracavernosal injection therapy, vacuum erection devices, penile occlusion devices, and for appropriate candidates, penile prosthesis implantation.
Low Testosterone
Hypogonadism or low testosterone is characterized by reduced energy, decreased libido, erectile dysfunction, mood changes, loss of muscle mass, and cognitive fog. As a low testosterone specialist serving York Region, Dr. Green diagnoses hypogonadism through a combination of laboratory evaluation and a comprehensive metabolic assessment to rule out secondary causes.
Testosterone replacement therapy (TRT) — available in oral medication, topical gels and intramuscular injections — can significantly improve symptoms for men with clinically confirmed hypogonadism. Dr. Green monitors patients on TRT with regular follow-up bloodwork and adjusts therapy based on clinical response and laboratory values, ensuring safe and effective long-term management.
Peyronie's Disease
Peyronie's disease is a condition in which scar tissue (plaque) forms within the erectile tissue of the penis, causing penile curvature, pain during erections, and often erectile dysfunction. Many men feel embarrassed to seek help, but Peyronie's disease treatment in Newmarket is available and effective — especially when initiated early.
For men in the acute phase (active plaque formation), treatment options include oral medications, intralesional injections, and penile traction therapy. Men with stable, severe curvature affecting sexual function may benefit from surgical correction — either plication procedures or plaque incision with grafting. Dr. Green will guide each patient through the evidence and select the most appropriate intervention.
Ejaculatory Dysfunction
Ejaculatory dysfunction encompasses several distinct conditions — premature ejaculation (PE), delayed ejaculation, anejaculation, and retrograde ejaculation. Each has different underlying mechanisms and requires a tailored treatment approach.
Premature ejaculation is the most common male sexual complaint and often responds well to a combination of behavioral techniques, topical desensitizing agents, and pharmacological therapy (SSRIs, dapoxetine). Retrograde ejaculation — where semen is directed into the bladder rather than expelled — is frequently associated with diabetes, certain medications, neurologic disorders or prior pelvic surgery, and may be managed medically or surgically depending on etiology. Dr. Green works collaboratively with patients to restore ejaculatory function and, when fertility is a concern, coordinates care with reproductive specialists.
