Enlarged Prostate (BPH)
Benign prostatic hyperplasia (BPH) is a non-cancerous enlargement of the prostate gland that affects the majority of men over age 50. As the prostate grows, it can compress the urethra and obstruct urine flow, causing a range of lower urinary tract symptoms (LUTS): weak or interrupted stream, straining to void, frequency, urgency, nocturia (waking at night to urinate), and a sensation of incomplete bladder emptying.
Patients with BPH are evaluated through clinical evaluation using symptoms assessment, digital rectal examination, PSA testing, urinalysis, and uroflowmetry. Medical management options include alpha-blockers (tamsulosin, alfuzosin) and/or PDE5 inhibitors (tadalafil 5 mg), which relax the prostate and bladder neck to improve flow, and 5-alpha-reductase inhibitors (finasteride, dutasteride), which shrink the prostate over time. Combination therapy is often more effective than either drug class alone.
When medications are insufficient or not tolerated, surgical options are considered. Dr. Green performs laser transurethral resection of the prostate (Greenlight TURP) and other minimally invasive procedures to relieve obstruction. He works with each patient to weigh symptom burden, medication side effects, and surgical risk to determine the optimal management strategy.

Symptoms include persistent pelvic, perineal, or genital pain lasting three or more months, often accompanied by urinary symptoms and, in some cases, sexual dysfunction. The condition can profoundly impact quality of life and may be misdiagnosed or undertreated.
